• 90792: Initial Psychiatric Evaluation (120 minutes)

• 99205: Medication Consultation (90 - 120 minutes)

• 99215 + 90833 (Medication Management and Psychotherapy; 30 minutes)

• 99215 + 90836 (Medication Management and Psychotherapy; 50 minutes)

Upon request, I will provide a statement that contains the information usually needed to process insurance claims.
